Advocate Hospice information

What are the key skills and qualifications needed to thrive as a Hospice Advocate, and why are they important?

To thrive as a Hospice Advocate, you need a strong understanding of end-of-life care, patient advocacy, and knowledge of hospice regulations, often supported by a healthcare or social work background. Familiarity with electronic health records (EHRs), case management systems, and compliance documentation is typically required. Exceptional interpersonal skills, compassion, and the ability to communicate sensitively with patients and families are crucial soft skills. These competencies ensure patients receive dignified care, families feel supported, and all legal and ethical standards are met during challenging times.

What are some common challenges faced by hospice advocates, and how can they effectively address them?

Hospice advocates often encounter challenges such as navigating sensitive conversations with patients and families, coordinating care among multidisciplinary teams, and managing emotional stress associated with end-of-life situations. To address these, advocates should cultivate strong communication skills, maintain empathy and professionalism, and utilize support from colleagues and supervisors. Regular training in grief counseling and clear documentation practices also help hospice advocates provide compassionate, effective care while maintaining their own well-being.

What are Advocate Hospice professionals?

Advocate Hospice professionals are healthcare workers who specialize in providing compassionate care and support to terminally ill patients and their families. Their focus is on improving quality of life, managing pain and symptoms, and offering emotional, spiritual, and practical support during the end-of-life journey. They work as part of an interdisciplinary team that may include doctors, nurses, social workers, chaplains, and volunteers. Advocate Hospice professionals also help families navigate difficult decisions and provide bereavement support after a loved one passes away.

Job description

Patient Advocate (Hospice Liaison) – Ovation Hospice | Utah

At Ovation Hospice, we believe every patient and family deserves exceptional end-of-life care — and that the person who connects them to that care carries one of the most meaningful roles in our organization.

We are looking for Patient Advocates who are driven, self-directed, and genuinely moved by this work. This is a field-based relationship role: you will be out in the community every day, building trust with physicians, hospital discharge teams, skilled nursing facilities, and case managers — and serving as the bridge between families navigating an incredibly difficult time and the care they deserve.

This is not a transactional sales position. It requires someone who can hold both the relational and the professional simultaneously — who understands how to build a referral territory from the ground up, stay accountable to their own numbers, and show up with warmth and credibility in environments where the stakes are high.

Cohort hiring note: We are currently hiring this role as a cohort across all three markets simultaneously. Selected candidates will share the same start date, onboarding experience, and compensation structure. We are building this team the right way — together.

What you will do

  • Build and manage a referral territory — cultivating relationships with physicians, hospital case managers, SNF social workers, and other healthcare partners

  • Educate referral sources on Ovation's clinical capabilities, patient criteria, and the value of timely hospice referrals

  • Meet with families to explain hospice benefits and help them understand their options during an emotionally complex time

  • Drive admissions growth by consistently generating qualified referrals and converting them to patients in care

  • Maintain accurate activity tracking in CRM — logging visits, contacts, and pipeline with discipline

  • Collaborate closely with your Administrator, clinical team, and intake coordinators to ensure seamless hand-offs and patient experience

  • Represent Ovation in the community with professionalism, humility, and genuine care for the people you serve

What we are looking for

We are not just screening for a resume. We are looking for a specific kind of person. The right candidate will recognize themselves in most of the following:

  • You build trust quickly — in a hospital hallway, a SNF conference room, or a family's living room, you know how to show up and earn credibility fast

  • You are self-directed — you plan your own week, work your own territory, and do not need someone to tell you where to be or when to follow up

  • You are resilient — a cold week or a lost referral does not derail you; you reset and keep going

  • You are motivated to win — you track your own numbers, you know where you stand, and you care about hitting your goals

  • You are aligned with the mission — you understand that hospice care is a gift, and you feel the weight and the honor of being the person who helps families access it

Prior experience in hospice, home health, healthcare sales, case management, or a community liaison role is strongly preferred. We will also consider candidates from outside healthcare who demonstrate exceptional relational drive and a genuine connection to this work.

COMPENSATION AND BENEFITS

  • Competitive Base Salary: $70,0000 - $90,000

  • Competitive Bonus Plan: Performance-based

  • Mileage Stipend and Cell Phone Reimbursement

  • Comprehensive Benefits: Medical, dental, vision, PTO, 401(k) retirement plan

Our hiring process

We run a structured, intentional process — out of respect for your time and ours. Every candidate goes through the same stages:

  • Written application question

  • Phone screen with a market leader

  • Structured in-person interview

  • Short behavioral assessment

  • Field ride with an Ovation leader (2–3 hours)

  • Final hiring decision

The field ride is mandatory and is one of the most important parts of our process. We want to see how you show up in the real environments where this work happens — and we want you to see what you are walking into. This is as much for you as it is for us.

About Ovation Hospice

Ovation Hospice is a multi-market hospice provider operating in Phoenix, Denver, and Salt Lake City. We are building something intentional — a company where clinical excellence, operational discipline, and genuine culture are not in tension with each other. We believe great care starts with the right people, supported by strong systems and real leadership.

We are growing — and we are hiring people who want to grow with us.

Ready to apply?

Complete the application and the prescreen survey that will be sent to you.  This is important as we usually only review applicants who complete the survey. Instead of a cover letter, please submit a brief response to the following question: "Describe a time you had to build a relationship from scratch in a difficult environment. What did you do?"
