Job Description:

Position Overview:

Under physician supervision, the Advanced Practice Provider - Inpatient is responsible for the comprehensive medical management of assigned inpatients in the hospital setting. The mid level provider is expected to exercise prudent judgment in the assessment, diagnosis, and implementation of care and seek appropriate collaboration as necessary.

*This Advance Practice Provider - Inpatient role will be working part-time at in Grand Rapids locations. The hours to work are nights from 7pm to 7am, guaranteed 24 hrs/week, every 3rd weekend.*

Desired/Required Qualifications:

Education:

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program

Experience:

  • Holds current BLS card for Health Professionals

  • Credentialed per CHC policies and procedures

  • Certified as a Physician Assistant or Nurse Practitioner

  • Inpatient hospital experience preferred

  • Licensed in the State of Michigan

Primary Duties and Responsibilities

1. Obtains comprehensive histories and physical examinations that reflect the acuity and presenting complaint of the patient. This includes daily rounding, admission and discharges that include comprehensive progress notes that reflect the acuity and priority needs of patients.

2. Coordinates and deliver comprehensive medical care, in collaboration with attending physicians, including, writing orders/prescriptions for standard medications, ordering and interpreting laboratory and radiographic studies, and seeking consultation as needed for optimal patient outcomes. This includes managing daily changes in patient status that requires more immediate resolutions.

3. Communicates in a timely and effective manner with supervising physician(s) as needed for continuity of care and optimal patient outcomes. Maintains communication with interdisciplinary teams including office services, hospital support services as well as additional physician (and physician teams) that may be also caring for the patient. Acts as a resource and responds to calls, in-house emergencies in a professional and timely manner.

4. Serves as a patient advocate and provides patient education and/or referrals for optimal outcome and continuity of care.

5. Works collaboratively with others across team lines by demonstrating accountability as a team member.

6. Performs other tasks, procedures, and/or duties within his/her scope of practice as assigned.
