Job description

Position Overview

We are excited to invite a committed and collaborative Advanced Practice Provider (APP) to become a part of our Pulmonary team in an outpatient-only environment. This position emphasizes delivering exceptional, patient-focused care in collaboration with our Pulmonary and Critical Care physicians, ensuring continuity for patients dealing with various respiratory issues.

Key Responsibilities
  • Deliver thorough outpatient care for pulmonary patients, encompassing evaluation, diagnosis, and management of both acute and chronic respiratory ailments.
  • Work closely with Pulmonary and Critical Care physicians to oversee shared patient panels.
  • Conduct follow-up appointments, manage medications, and adjust care plans as necessary.
  • Facilitate care coordination across different specialties, including referrals, testing, and procedures.
  • Provide education to patients and their families regarding disease processes, treatment options, and preventive measures.
  • Analyze diagnostic tests such as pulmonary function assessments, imaging studies, and laboratory results.
  • Ensure accurate and timely documentation within the electronic medical record system.
