Job description

Clement Manor is a faith-based independent living, assisted living, CBRF and skilled long-term care community, nationally recognized for providing a superior quality environment for older adults.
We have exciting opportunity for a Social Worker to join our team. This individual will manage a caseload of approximately 50 residents in our skilled, memory care and transitional care departments.
Key responsibilities include:
  • Welcome new residents and family members and orient them to Clement Manor
  • Coordinate assessments and care plans
  • Charting/Documenting appropriate clinical notes
  • Audit clinical records
  • Advocate for residents in the investigation resolution of potential resident rights violations
  • Discharge planning
  • Assist resident with external resources and applications
  • Use verbal and nonverbal communication reflecting our values of respect, collaboration and quality service

Ideal candidates will have a Bachelor's degree with a major in social work, psychology, sociology or related field as well as a current social worker license for the state of Wisconsin. Preferred experience includes previous experience in a similar setting, experience with discharge planning and exposure to MDS.
