What will Flexible Scheduling Offer Me?
•    Extended Hours Monday-Friday starting 7:30a-7p
•    Extended Hours Saturday & Sunday starting 7:30a-7p
•    Nurse Assessor Can Build Their Own Schedule Each Week
•    Nurse Assessor Can Make Up Hours for Interrupted/No Shows
•    Work Any Calendar Day
Pay Rate: $58-$60/hr.
Hourly wage may depend upon experience, education, geographic location and other factors.
Qualifications
• New York State RN License, required.
• Work experience with UAS-NY or Oasis Community Assessment tool
• 2+ years’ experience working in-home setting within the last 15 years, required.
• Work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities.
• Computer literacy: ability to navigate new and emerging technology; ability to perform telehealth assessment; highly developed written and oral communication skills; strong interpersonal skills; and proven ability to work with community groups.
• Knowledge of long-term care services and programs
• Ability to travel throughout the assigned region, as assigned, including travel to individuals’ homes.

Responsibilities
• The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, to assist the Physician in the completion of a Physicians order.
• The incumbent may follow up with individuals who have received an assessment through face-to-face communications.
• Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports.
• Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes.
• Work collaboratively to monitor and address clinical quality and gaps in care.
• Provide clinical and educational assistance to support project initiatives.
• Demonstrate communication skills necessary to interpret age-specific responses.
