Schedule: MondayFriday | 8:00 AM 5:00 PM MST

As an Intake Coordinator, youll play a vital role in supporting patients and families by coordinating services that help individuals recover, manage chronic conditions, and remain safely in the comfort of their own homes.

Key Responsibilities

  • Receive and process patient referrals by phone and in person
  • Explain home health services and benefits to patients, families, and referral sources
  • Assist in identifying alternative services when a patient is not eligible for care
  • Coordinate and schedule initial nursing assessments and admissions
  • Prepare and maintain admission packets and required documentation
  • Support community outreach efforts and maintain strong referral relationships
  • Provide administrative and clerical support to the clinical team
  • Maintain accurate patient records and documentation in a timely manner


Qualifications

  • Must be at least 18 years of age
  • One (1) year of experience in home health, healthcare, or a related field preferred
  • Strong organizational, communication, and customer service skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Ability to complete documentation accurately and on time
  • Valid drivers license and proof of auto insurance required
