Kline Galland Hospice is seeking an organized, detail-oriented, and compassionate Hospice Team Coordinator to support our interdisciplinary hospice team. This is a non-clinical administrative role that plays a key part in ensuring smooth daily operations, regulatory compliance, quality initiatives, and coordinated patient care delivery.

The Hospice Team Coordinator provides administrative, operational, quality, and compliance support to hospice leadership and team members. This role assists with scheduling coordination, onboarding activities, reporting, documentation management, quality improvement initiatives, and regulatory compliance monitoring.
The Coordinator maintains key tracking systems, supports workflow efficiency, facilitates communication across the hospice program, and serves as a backup resource for Intake and selected Office Manager responsibilities to help ensure continuity of operations.
What You'll Do
Administrative & Operational Support
  • Provide day-to-day administrative support to hospice leadership and team members.
  • Maintain operational tracking systems, reports, departmental resources, and shared tools.
  • Prepare hospice invoices and support billing-related administrative functions.
  • Provide backup support for payroll-related processes.
  • Assist with special projects and workflow coordination.
Scheduling & Care Coordination Support
  • Assist with scheduling visit requests across hospice disciplines.
  • Coordinate and communicate daily schedule changes.
  • Support care coordination activities to ensure efficient service delivery.
  • Collaborate with team members to resolve scheduling and workflow concerns.
New Hire & Team Support
  • Prepare clinical bags, forms, and orientation materials for new employees.
  • Coordinate orientation schedules in partnership with hospice leadership.
  • Initiate technology access requests and onboarding processes.
  • Track onboarding requirements, including TB compliance.
  • Assist with onboarding logistics and scheduling.
Clinical Support (Non-Clinical Functions)
  • Create hospice aide and comfort therapy assignments and service orders as directed.
  • Maintain hospice aide and comfort therapy census tracking.
  • Follow up regarding hospice aide care plans and required documentation.
  • Conduct care plan audits and support corrective actions.
  • Monitor visit-order compliance and MatrixCare task completion.
  • Track recertification documentation, medication audits, and IDT signatures.
  • Assist with documentation accuracy and operational workflow support.
Quality, Compliance & Audit Support
  • Complete HOPE tool audits and submissions.
  • Support QAPI activities, data collection, tracking, and follow-up.
  • Assist with CAHPS processes, caregiver information collection, and feedback tracking.
  • Support infection surveillance and end-of-life reporting activities.
  • Track unmet needs reporting and extended respite reporting.
  • Ensure compliance with HIPAA and organizational policies.
Reporting & Data Management
  • Generate and maintain reports including:
    • Interdisciplinary team caseload reports
    • Hospice aide caseload reports
    • Monthly triage reports
    • Tuck-in reports
    • Visit-help tracking reports
  • Create weekly IDT lists and supporting documentation.
  • Manage discharge, census, and active patient reports.
  • Update Bellevue Healthcare discharge tracking.
  • Audit billing summary reports for accuracy.
Office Manager Backup Support
Provide backup assistance with:
  • Payroll administration
  • Relias training tracking
  • TB compliance tracking
  • Performance evaluation tracking
  • Non-clinical QAPI activities
Communication & Team Coordination
  • Support effective communication among hospice team members and departments.
  • Coordinate and prepare materials for IDT meetings.
  • Distribute reports and care coordination communications.
  • Provide timely follow-up on operational and administrative needs.

Position Qualifications
  • High school diploma or equivalent required; additional healthcare or administrative education preferred.
  • Experience in hospice, home health, healthcare administration, or a related setting preferred.
  • Strong organizational, time management, and multitasking skills.
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office applications and electronic medical record systems (e.g., MatrixCare).
  • Ability to prioritize competing responsibilities and meet deadlines in a fast-paced environment.
