• Conduct thorough assessments to identify clients' social, emotional, and environmental challenges that may impact their mental health.
• Develop and implement personalized service plans that define clear goals, interventions, and methods for tracking progress.
• Engage in individual and family counseling sessions, utilizing various therapeutic modalities to guide clients through challenges.
• Provide crisis intervention, including risk assessment and safety planning, ensuring the well-being of clients in distress.
• Collaborate seamlessly with clients and their families to build robust connections to community resources, ensuring comprehensive support.
• Network with local agencies and organizations, advocating for clients’ needs and coordinating services across various sectors.
• Engage in proactive community outreach to raise awareness about the services offered by Nurturing Minds Counseling.
• Participate actively in case conferences, team meetings, and ongoing staff training units to enhance professional development and agency efficacy.

This role primarily serves individuals and families facing critical social, emotional, and mental health issues. With an emphasis on those in hospice care, you will work directly with clients navigating the complexities of end-of-life issues, grief, and mental well-being. By employing compassionate therapeutic techniques, you will provide invaluable support to those experiencing some of life's most challenging moments.

• A Master’s degree in Social Work from an accredited institution is mandatory.
• Must possess an active state license as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of trauma-informed care principles, social justice issues, and community resources.
• Exceptional communication skills, cultural competency, and problem-solving abilities to engage diverse populations effectively.

• Previous experience in a social service agency or similar therapeutic setting is highly valued but not required.
• Familiarity with evidence-based therapeutic practices and crisis intervention strategies is a significant advantage.
