This position is based in Fontana, California, but it offers the flexibility of a remote work setting, with opportunities to engage in outpatient office visits as needed. • Conduct thorough assessments, create diagnostic evaluations, and monitor the effectiveness of mental health care interventions, utilizing tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM) to tailor care strategies.

• Collaborate with patients and their families to construct comprehensive treatment plans that prioritize patients' unique needs, cultural backgrounds, and treatment goals.

• Continuously evaluate patient progress and implement evidence-based interventions to effectively support mental health diagnoses, fostering positive outcomes and symptom reduction.

• Maintain accurate and timely treatment records, clinical documentation, and reports, ensuring compliance with regulatory standards and organizational policies.

• Act as a liaison for referrals, forging strong relationships with other healthcare providers to ensure seamless care transitions for patients.

• Facilitate educational sessions and mentoring for students, interns, and volunteers, contributing to the professional development of future mental health practitioners.

• Engage in peer reviews and clinical collaborations under the guidance of supervisors to promote team growth and efficacy.

The LCSW will work with a diverse patient population, addressing a range of mental health issues from anxiety and depression to more complex psychological conditions. • A Master’s degree in Social Work from an accredited institution.

• Experience working in outpatient settings, particularly with telehealth services.

• Proficiency in using electronic health record (EHR) systems to manage patient documentation and data.

• A history of collaborating across multidisciplinary teams to enhance patient care delivery.
