Job description

Are you passionate about making a meaningful impact in your community by supporting individuals facing serious mental health challenges? Join a dedicated Assertive Community Treatment (ACT) Team that delivers intensive treatment, rehabilitation, and support services to those with significant behavioral health needs who have not found success with traditional care. As an ACT Program Assistant, you will provide essential clerical and office support to ensure high-quality, patient-centered care for consumers served by the team.
Key Responsibilities:

  • Manage all telephone inquiries related to the ACT Team, triaging messages while maintaining confidentiality.
  • Communicate effectively with team members, agency staff, and community partners.
  • Screen client contacts for crisis situations, prioritizing and routing appropriately.
  • Schedule and cancel client appointments accurately on a daily basis.
  • Participate in daily team huddles by taking notes and sharing relevant client information.
  • Collect and report aggregate data on client contacts for ongoing review.
  • Track and report client hospitalizations and incarcerations.
  • Maintain up-to-date client records and coordinate with the team to collect missing information.
  • Conduct electronic chart audits to ensure compliance with team and agency standards.
  • Maintain and update program materials, forms, and office supplies.
  • Complete all required documentation, training, and competency reviews promptly.
Education:
  • High school diploma or GED equivalent required.
Experience:
  • Preferred: Two or more years of professional experience in mental health services, crisis response, or related fields.
  • Experience educating and teaching clients about self-care and improving daily living activities is a plus.
Requirements:
  • Computer proficiency.
  • Must pass background checks including child and adult abuse registries and motor vehicle screening.
  • Completion of CPR and First Aid training within 60 days of employment.
  • Valid driver's license, insurance, and reliable vehicle.
