About the Role:
Center Street Community Health Center is seeking a compassionate and dedicated Family Practice Nurse Practitioner to serve as the primary provider in our new School-Based Health Center located within the Marion City School District. This innovative health center will provide accessible, high-quality primary care services to Marion City Schools students and staff while also serving members of the broader community.
The Family Practice Nurse Practitioner will provide comprehensive primary care across the lifespan, including preventive care, acute and chronic disease management, health screenings, health education, and care coordination. As the lead clinical provider for this new site, the Nurse Practitioner will play a key role in establishing workflows, building relationships with school personnel and community partners, and helping shape a patient-centered model of care that improves access to healthcare services and supports healthier outcomes throughout the community.
This position offers a unique opportunity to make a lasting impact by helping launch a new healthcare access point that addresses the needs of students, families, school employees, and community members in Marion County. This position may have reduced hours in the Summer.ย 
Requirements:
  • Master's degree in Nursing with a focus on Family Practice.
  • Current Nurse Practitioner license in the state of Ohio.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively in a team-oriented environment.
