Responsibilities

Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care. The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse. This position provides direct patient care to pediatric, adolescent, adult and geriatric patients.

If you meet these qualifications, we would love to meet you:

  • Graduate of an accredited practical or vocational nursing program
  • Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
  • One year of LVN/LPN work experience
  • Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver who can travel to all business locations

Responsibilities:

As a LVN/LPN, you will:

  • Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
  • Promotes and maintain optimal client care under the supervision of an RN.
  • Maintains a safe patient environment.
  • Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members.
  • Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician.
  • Re-evaluates client’s nursing needs and reports findings to Case Manager.
  • Counsels and instructs the client and family in meeting nursing and related needs.
  • Prepares clinical and progress notes.
  • Submits all documentation in a timely manner in accordance with agency policy.
  • Documentation on patients chart correlates with nursing care plan.
  • Communicates frequently and works closely with other home care team members.
  • Assists in coordinating services provided to client
