Job Title

Registered Nurse - After Hours
Job Description Summary
Reports to: Clinical Manager or Director of Clinical Services (if Clinical Manager is not yet established)
The Registered After-Hours Nurse is responsible for providing direct patient care during evening and designated coverage hours. Responsibilities include rapid response to patient, family, facility, and caregiver need, in alignment with the hospice plan of care and organizational standards.
Roles & Responsibilities
Provides direct patient care as permitted by the State Nurse Practice Act, including specialized duties related to end-of-life care.
Responds promptly to patient, family, and facility requests occurring primarily during evening or after-hours periods.
Coordinates with the Director of Clinical Services (DCS) or RN Case Managers regarding needed nursing services during coverage hours.
          a. Immediately notifies DCS or Administration of any service concerns or issues.
Contacts the Hospice Medical Director for physician orders related to medications, treatments, or symptom changes.
When required, performs pronouncement of death.
          a. Provides support, empathy, and guidance to families and facilities at the time of death.
          b. Completes all required documentation for death visits.
Documents all occurrences such as falls, medication errors, infections, and complaints with the EMR.
Implements nursing interventions in alignment with current standards, the comprehensive assessment, and the established plan of care.
Provides accurate, timely documentation to reflect care provided and changes in condition.
Educates patients and families/caregivers regarding diagnosis, symptom management, and care strategies appropriate to their needs.
Coordinates with the interdisciplinary group and reports identified need that may require involvement from other disciplines such as hospice aide, OT, PT, SW, ST, Dietician, or Director of Clinical Services.
Uses and prepares equipment and supplies properly, following aseptic technique.
Provides appropriate pain and symptom management and evaluates patient responses to interventions and medications.
Participates in personal and professional development and takes part in quality assessment and performance improvement (QAPI) activities.
Performs additional duties as assigned by the RN, including:
             a. Assisting with processing and completing assigned clinical audits.
             b. Completing after-hours admissions when required.
Qualifications
Graduate of an accredited Registered Nurse or vocational nursing program
Three years of nursing experience preferred; hospice or medical-surgical experience strongly preferred. 
Currently licensed as Registered Nurse in the state. 
Complies with accepted professional nursing standards and practices. 
Demonstrates strong verbal and written communication and organizational skills. 
Must be a licensed driver with a reliable automobile that is insured according to the state and organizational requirements. 
Possesses and maintains current CPR certification. 
Understands hospice philosophy and the unique needs of the terminally ill.
