Department: LTC | CNA
*Required Certifications: BLS*
- # of Beds/Residents: 34 beds- currently 22 residents. All Semi Private Rooms.
- EMR: Cerner
- Ratios: RN's/LPN's: Days 1:11, nocs 1:22 or 1:30, CNA's: days 1:6, nocs 1:6 or 1:8.
- Required Certifications: BLS
- Common diagnosis: COPD, DM, cognitive deficits, comorbidities, etc.
- Job Duties: RNs/LPNs: Prefer leadership experience such as charge. Nurses pass med, Simple IVs and tubes. PT manages-
wound care, nurses as needed. No vents or trachs.
- Experience: Prefer experience with cognitive deficits; Need to understand how to interact with cognitive deficits
and know how to redirect a person and not argue. LPNs do NOT need to be IV Certified. Will accept first time travelers.
- Are there IV's or titratable drips?: None
- Tele monitors: None
- Are RN's required to attend team conferences for acute inpatient rehab?: None
- Charting System: Cerner
- DON/Charge Nurse/House Supervisor: Days: RCC, MDS, IP/Ed, DON. Nocs-Charge nurse available from other
departments. DON available by phone. RN on every shift which could be someone from the leadership team.
- Physician availability after hours: Yes. NP available M-Fr and by-call in the after hours, MD oversight.
- Interdisciplinary Team: PT/OT/SLP, DON, Resident Care Coordinator, MDS Coordinator, Resident Advocate, Dietician,
Finance, NP, Medical Director if needed.
- Is RT on-site 24/7. Oxygen Therapy /Equipment: ER available 24/7; Nurses to administer all Resp Therapy and Home Cpap
& Bipap.
- Pharmacy on-site: Pharmacy on site, on call on nocs. Omnicell utilized to pull medications; No bubble packs.
- Shifts & Scheduling: 12 Hr Shifts from 7-7:30. Staffing: Days: 2 nurses and 4 cnas, Nocs:1 nurse and 3-4 cnas. Modified
Self Scheduling, travelers will be asked to submit their availability, however they are asked to stay flexible and help fill
gaps in permanent staffing. Schedule is not finalized until it has been balanced by the manager. Do NOT guarantee block
scheduling.
- Weekend Requirements: Varies but typically EOW.
- Holiday Requirements: May be asked to go into the Holiday Rotation.
- Call or Standby Requirement: None
- Floating Requirements: To memory care unit, if needed.
- Scrub Color: RN's: Navy Blue, CNA's: Any Color.
- Unit Specific Orientation: 1 shift of Orientation with a preceptor.
