Job description

Verity Behavioral Services & Primary Care is seeking a compassionate, organized, and client-focused Case Manager to support children, adolescents, and adults receiving behavioral health and supportive services. The Case Manager will assess client needs, coordinate community resources, and advocate for individuals and families to ensure continuity of care and improved outcomes.

This role works closely with clinicians, families, schools, medical providers, and community partners to address psychosocial, educational, and functional needs.

Key Responsibilities

  • Conduct comprehensive psychosocial and needs assessments

  • Develop, implement, and monitor individualized service plans

  • Coordinate care with therapists, psychiatrists, primary care providers, schools, and community agencies

  • Link clients to community resources (housing, education, medical care, food assistance, employment, etc.)

  • Advocate for clients and families to ensure access to appropriate services

  • Monitor progress toward treatment goals and adjust plans as needed

  • Maintain accurate, timely, and compliant documentation in the electronic health record (EHR)

  • Participate in treatment team meetings and case conferences

  • Provide crisis intervention support and referrals when necessary

  • Ensure compliance with Ohio Medicaid, agency policies, and ethical standards

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Human Services, Sociology, or related field (required)

  • Case Management experience in behavioral health, mental health, or social services (preferred)

  • Knowledge of community resources and social service systems

  • Strong organizational, communication, and documentation skills

  • Ability to work independently and collaboratively as part of a multidisciplinary team

  • Experience working with children, adolescents, and/or families preferred

  • Valid driver’s license and reliable transportation (if applicable to role)

Preferred Certifications (Not Required)

  • Licensed Social Worker (LSW)

  • Ohio Medicaid Case Management experience

  • Experience with mental health day treatment or community-based services
