Job Summaryย ย
The Patient Care Coordinator supports Total Care Connectโsย Mobile Integrated Healthcare (MIH) programs by coordinating referrals, managing patient care workflows, and ensuring seamless communication between healthcare partners, Community Paramedics, and care management teams in a call-center type environment.ย
This role is responsible for processing partner referrals, supporting the coordination of patient visits, and facilitating appropriate followโup after care is delivered. Patient Care Coordinators help ensure continuity of care by identifying and relaying care needs, coordinating postโvisit tasks, and distributing visit documentation such as After Visit Summaries (AVS).ย
The role also includes assisting with transitionโofโcare calling for other teams when needed to fill calling gaps, ensuring patients receiveย timelyย followโup during care transitions.ย
Hourly: $22 - 26/hr

Qualificationsย ย

โข Experience in healthcare operations, patient coordination, medical office administration, or a related fieldย
