Job description

Location: Nursing Homes in Ohio

Position Overview

Moxie Health is seeking a dedicated and experienced Wellness Assistant to join our team at our Nursing Homes in Ohio. As a Wellness Assistant, you will play a crucial role in ensuring the smooth operation of our clinic and providing high-quality care to our patients.

Responsibilities
  • Serve as the primary point of contact for patients enrolled in Remote Patient Monitoring (RPM) programs

  • Collect, record, and review patient data from monitoring tools (e.g., vital signs, glucose levels)

  • Identify trends and abnormalities in RPM data and escalate findings to healthcare providers to support care plan adjustments

  • Provide clinical support for diagnostic procedures, including POCUS, spirometry, and stress testing, ensuring patient comfort and protocol compliance

  • Educate patients on RPM programs and how to understand and use their health data effectively

  • Collaborate closely with healthcare providers and clinical teams by sharing RPM insights to support patient care strategies

  • Promptly communicate abnormal vital signs and significant clinical changes to appropriate healthcare providers

  • Adhere to all facility policies and procedures, including HIPAA and Joint Commission standards

  • Do not engage in direct patient care activities outside the defined role

  • Redirect patient care–related questions and clinical decision-making to the responsible licensed provider

Required Qualifications
  • High school diploma or equivalent

  • Completion of a Medical Assistant program from an accredited institution

  • Certification as a Medical Assistant (CMA)

  • Previous experience working as a Medical Assistant in a clinical setting

  • Knowledge of medical terminology, clinical procedures, and protocols

  • Excellent interpersonal and communication skills

  • Strong attention to detail and organizational skills

  • Ability to multitask and work efficiently in a fast-paced environment

  • Basic computer skills and proficiency with electronic health record (EHR) systems

  • Bilingual proficiency in English and Spanish preferred
