Overview


Join our compassionate healthcare team as a Home Health Aide, where your dedication and caregiving skills will make a meaningful difference in the lives of patients receiving home and community-based care. In this vital role, you will provide personalized assistance to individuals with diverse needs, promoting independence, comfort, and well-being in their own homes. Whether supporting seniors, individuals with disabilities, or those recovering from illness, your efforts will foster a safe and nurturing environment that encourages dignity and quality of life.


Responsibilities

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or transfer belts.

  • Support patients with specialized care needs such as Alzheimer’s disease, dementia, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities.

  • Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment that meets health standards.

  • Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans.

  • Utilize medical terminology confidently when recording observations or communicating with medical professionals to ensure clarity and accuracy.

Skills

  • Proven experience in home health care or long-term care environments with knowledge of medical procedures and documentation practices.

  • Strong patient observation skills to detect changes in condition promptly and accurately.

  • Experience working with individuals with disabilities or in assisted living settings is highly valued.

  • Knowledge of medical terminology along with CPR certification to respond effectively in emergencies.
