What you'll do:


The OhioRISE Navigator is a mission-critical role that strengthens system coordination, improves access to care, and enhances outcomes for youth and families by ensuring timely, efficient, and well-supported transitions from inpatient to community-based services.


Minimum Qualifications:

Bachelor’s degree in social work, healthcare administration, nursing, or related field and/or equivalent experience

Minimum of 2–3 years of experience working in behavioral health, healthcare systems, or care coordination

Knowledge of hospital systems, emergency departments, and discharge planning processes

Strong organizational, communication, and problem-solving skills


Preferred Qualifications:

Experience with OhioRISE, Medicaid systems, or managed care organizations

Experience working with inpatient psychiatric or medical units

Knowledge of Aetna MAF processes, authorization workflows, or referral management

Experience in care transitions, utilization management, or system navigation

Experience providing education or training to providers or staff
