We are seeking an experienced and detail-oriented MDS Coordinator to join our skilled nursing team in Delaware, Ohio. This is an excellent opportunity for a nurse who is passionate about resident-centered care, clinical excellence, and maximizing reimbursement accuracy through effective MDS management.
As the MDS Coordinator, you will play a key role in coordinating the Resident Assessment Instrument (RAI) process, ensuring regulatory compliance, and collaborating with an interdisciplinary team to deliver exceptional outcomes for residents.
What You'll Do
  • Coordinate and complete MDS assessments in accordance with federal and state regulations
  • Manage the RAI process and ensure timely completion of assessments and care plans
  • Monitor Medicare, Managed Care, and Medicaid reimbursement opportunities
  • Collaborate with nursing, therapy, dietary, social services, and other departments to ensure accurate documentation
  • Conduct chart reviews and audits to support compliance and reimbursement accuracy
  • Participate in care plan meetings and interdisciplinary team discussions
  • Stay current with regulatory changes and reimbursement guidelines
  • Support quality measures and clinical outcomes initiatives
Qualifications
  • Current RN license in the State of Ohio
  • Previous MDS Coordinator experience preferred
  • Strong understanding of RAI/MDS processes and Medicare reimbursement systems
  • Knowledge of PDPM and skilled nursing facility regulations
  • Excellent organizational, analytical, and communication skills
  • Ability to work independently while collaborating effectively with interdisciplinary teams
