Skilled Nursing / Long-Term Care Full-Time
Creekside Care Center of Gahanna is seeking a motivated and compassionate Full-Time Physical Therapist Assistant (PTA) to join our dynamic rehabilitation team! If you are passionate about helping patients regain mobility, independence, and quality of life, we would love to meet you.
Our therapy team works in a supportive Skilled Nursing and Long-Term Care environment, using Net Health documentation and collaborating closely with interdisciplinary staff to deliver exceptional patient-centered care.

Responsibilities
  • Provide physical therapy treatments under the direction of a licensed Physical Therapist
  • Assist in implementing individualized treatment plans to improve mobility, strength, and function
  • Document patient progress and treatments accurately using Net Health
  • Communicate effectively with therapists, nursing staff, and interdisciplinary team members
  • Educate patients and caregivers on therapeutic exercises and mobility techniques
  • Maintain compliance with facility policies and regulatory standards

Qualifications
  • Graduate of an accredited Physical Therapist Assistant (PTA) program
  • Current PTA license in the state of Ohio (or eligible for licensure)
  • Experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC) preferred, but not required
  • Strong communication and teamwork skills
  • Passion for improving the lives of patients and residents
