Key Responsibilities

• Act as a liaison between families and outpatient healthcare providers, ensuring cohesive communication and coordinated care for clients.
• Interact with insurance companies to facilitate pre-certifications and manage utilization requests, advocating for client needs throughout the insurance process.
• Participate actively in discharge planning and aftercare support, collaborating with a multidisciplinary team to ensure seamless transitions for clients returning to community resources.
• Provide therapeutic meal support and psychoeducation, promoting a healthy relationship with food and addressing the emotional aspects related to eating behaviors.
• Deliver empathetic and affirmative care that respects and understands each client's intersecting identities, promoting healing and growth in their individual contexts.

Patient Population / Client Focus

This role primarily focuses on older adults, recognizing the specific psychological and social challenges they face. Our clients may experience issues related to mental health conditions, loneliness, age-related transitions, and other psychosocial stressors. As a social worker, you will play a pivotal role in improving their quality of life by providing tailored support and fostering independence in navigating their social and emotional landscapes.

Qualifications

• Master's degree in Social Work from an accredited institution is required.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Florida is essential.
• Proven experience working with older adults, demonstrating an understanding of their unique needs and circumstances.
• Strong clinical and diagnostic skills, equipped to assess and address a range of psychological issues.
• Excellent communication and interpersonal skills, able to build rapport effectively with clients, families, and colleagues.
• Capacity to work both independently and collaboratively within a team-oriented environment, utilizing a range of modalities and interventions.

Preferred Experience

• Experience in community health settings or familiarity with programmatic approaches to mental health care.
• Knowledge of various treatment modalities, such as cognitive behavioral therapy (CBT), motivational interviewing, or other evidence-based practices.
• Understanding of insurance processes related to therapy services, including pre-authorizations and treatment plans.

Compensation and Benefits
