    Job description

    Advanced Practice Providers are key members of the Lifespark Medical Group primary care practice. We deliver primary care to frail and complex seniors living in a variety of settings including Community, independent living, assisted living, memory care, long term care, and transitional care units. We utilize a combination of in-person and telehealth visits inside a multidisciplinary team-based model. Clients and their caregivers receive expert, accessible, and compassionate care.
    This collaborative and dynamic medical practice is led by industry experts in geriatrics and senior care services. It is delivered under the umbrella of a larger Lifespark enterprise which provides whole person senior care to broad populations of seniors. Lifespark has value-based arrangements with payers which allows clinicians to practice independent of traditional healthcare and payment constraints. These payment methodologies allow clinicians to practice the way they always envisioned when they chose to join the caring profession. And you will be part of a team that focuses on the seniors who need you the most!
    This position requires deep understanding of what high value care means and what is required to deliver it inside the trusted relationship between clinicians, clients, families, and senior living staff. High clinical acumen in geriatrics, strong listening skills, a desire to mentor, high level of flexibility and robust organizational skills are required strengths.
    Position: Nurse Practitioner/Advanced Practice Provider
    Salary: $120,000 - $140,000 annually, depending on experience
    Service Area: Twin Cities Metro area. Plus, mileage reimbursement of 72.5 cents per mile.
    Schedule: 1.0 FTE, Full - Time / Monday - Friday, 8:00am-5:00pm (volunteer opportunities for on-call, no weekends, and holiday rotation within the team)

    Position Functions & Responsibilities:
    • Builds relationships with patients and caregivers to ensure high value outcomes are obtained.
    • Utilizes evidenced based practice guidelines, assessment tools, standards, processes to deliver best in class geriatric care.
    • Performs focused and comprehensive assessments to develop an overall health care plan.
    • Manages acute and chronic health conditions timely and effectively, based on member acuity.
    • Follows up on interventions, consults and changes in health treatment plan.
    • Documents accurately and timely to reflect the member's health status.
    • Collaborates with Lifespark team when member requires support, community services or other health related support services.
    • Communicates regularly with member, family, pertinent health services staff, including care conferences when appropriate.
    • Communicates with subspecialists and primary care providers in the community as needed
    • Integrates self into a team-based model of physicians, advance practice registered nurses, registered nurses and licensed practical nurses, social workers, home health workers and team assistants.
    • While on-call after business hour work is paid separately and designed to be voluntary, it may be required under some circumstances
    • Other duties as assigned.

    Qualifications:
    • New Grads with Acute Care experience is considered.
    • A current Minnesota APRN and RN license and DEA license
    • Certification in Geriatrics preferred OR strong experience required.
    • Board certification through ANCC or AANP
    • Meet threshold criteria/qualifications for Credentialing and Privileges.
    • Communicates effectively in verbal, written, and electronic methods.
    • Able to work in non-traditional settings (client homes, nursing homes, and from home)
    • Ability to develop strong and positive relationships with patients, caregivers, and team members
    • Comfortable with complexity.
    • Strong knowledge of medical management and commitment to use of evidence when it exists and acknowledgement when it does not.
    • Interest in managing problems holistically including attention to cognitive, functional and psychosocial domains
    • Strong communication skills
    • Creative problem solver
    • Strong customer service qualities including empathy, positivity, desire to serve, resourcefulness and reliability
    • Independently motivated yet possesses a collaborative spirit and desire to be part of a team
