ABOUT LIFEMOVES
LifeMoves is the largest and most effective provider of housing and services for neighbors experiencing homelessness in Silicon Valley since 1987. Our Mission is to end homelessness by providing interim housing, supportive services, and building collaborative partnerships. With 40 programs, LifeMoves gives our neighbors experiencing homelessness a temporary place to call home while providing intensive, customized case management through site-based programs and community outreach.
POSITION PURPOSE
LifeMoves | Healthcare Outreach programs provide critical support to unsheltered individuals, helping clients build a foundation for long-term stability. The teams work directly with individuals experiencing homelessness, providing case management, connecting them to medical care and community resources, and supporting them during transitions such as hospital discharge. All programs focus on improving health outcomes and increasing opportunities for housing stability.
The Healthcare Outreach Program Director (PD) reports to the Associate Director of Specialty Outreach Programs and provides oversight to staff in the field in the Healthcare for the Homeless (HCH) and Kaiser Outreach programs. HCH connects individuals in the community to medical care, the San Mateo County Street Medicine Team, and provides health insurance assistance. The Kaiser team works with emergency departments at four different Kaiser hospitals in Santa Clara and San Mateo Counties. This program aims to establish linkage with longitudinal healthcare services to reduce emergency department recidivism. This management position plays a key role in supporting daily operations, safety, and overall program success. They model the agency’s values by treating everyone with respect and care, creating a culture where clients and staff feel welcomed, empowered, and supported.
The Healthcare Outreach Program Director focuses on building strong team relationships, inspiring professional growth, and maintaining a healthy, positive program culture. They provide trauma-informed supervision and coaching to staff, partners with leadership to identify training needs, strengthen communication, and ensure consistent delivery of high-quality, client-centered services and accurate data input. Additionally, this role helps keep the program safe and supportive by working with staff and clients to identify concerns, provide education, and develop solutions. They maintain positive relationships with community partners and funders, ensure contract compliance, and lead with curiosity, creativity, and collaboration to move the program forward.

SUPERVISORY RESPONSIBILITIES
This role provides supervision to Case Managers and Specialists (as applicable: outreach, housing, employment, benefit, children service coordinators, licensed vocational nurses).
TRAVEL REQUIREMENTS
This position requires regular travel between agency sites, community partner locations, client service events, and internal and external meetings. As such:
PHYSICAL DEMANDS
This is an active position requiring frequent movement throughout residential facilities and surrounding property or out in the field in encampments and surrounding areas. Employees may regularly stand, walk, climb stairs, bend, kneel, reach, push, pull, carry supplies, move furniture, prepare spaces, transport client belongings, and perform indoor and outdoor safety rounds.
Depending on the assigned program, staff may work independently for portions of their job and should be comfortable responding to emergencies, making sound decisions, and adapting to changing situations until additional support is available. The work environment includes frequent interruptions, changing priorities, and regular interaction with individuals experiencing homelessness and other complex life challenges. Success in this role requires professionalism, adaptability, emotional resilience, and the ability to remain calm under pressure.
