We have Contract role Social Worker for our client at Clinton TN. Please let me know if you or any of your friends would be interested in this position.
Position Details:
Social Worker - Clinton, TN
Location                    : Clinton, Tennessee 37716
Project Duration   : 12 Months +
Salary Range           : $20/hr to $22/hr
Shift                             : 8:00am to 4:30pm
Job Summary
The Social Worker supports primary care providers and patients through case management, social support navigation, referral coordination, patient education, and community resource collaboration to improve continuity of care and patient outcomes.
Key Responsibilities
Social Support Navigation / SDOH
  • Assist patients with social determinants of health (SDOH) needs including food insecurity, housing insecurity, and access to community resources
  • Compile and maintain resource lists including eligibility criteria, referral processes, and contact information
  • Conduct in-person or remote social needs screenings/assessments
  • Collaborate with primary care nurses, providers, and social workers
  • Coordinate or connect patients with social services resources including housing, food, clothing, and mental health services
Case Management
  • Support discharge planning, follow-up, and patient education with hospitals
  • Assist with obtaining patient records from hospitals and specialists
  • Help secure medical equipment through community partners
  • Conduct follow-up on patient care plans
  • Identify patients overdue or lost to follow-up and assist them in returning to care
Referral & Prescription Assistance
  • Schedule, coordinate, and track specialist and imaging referrals
  • Maintain referral resource lists including eligibility, cost, and contact information
  • Assist patients with low-cost prescription resources and patient assistance programs
  • Support applications for programs such as Cover Rx and Rx Outreach
Documentation & Patient Support
  • Document in patient records and update consults as needed
  • Tag providers and clinical staff appropriately within records
  • Provide patient education and identify appropriate educational resources
 
Additional Expectations
  • Complete onboarding and orientation requirements
  • Participate in clinical, provider, and regional office meetings as requested
  • Attend Health Councils and community meetings to build relationships with social service agencies
  • Identify barriers to patient care and assist in developing solutions
