Key Responsibilities
  • Engage with assigned clients, providing clinical assessments and therapy aimed at managing symptoms of mental illness and emotional disturbances.
  • Develop and implement tailored care plans that integrate services from various providers, ensuring comprehensive support for clients and their families.
  • Perform evidence-based screenings and assessments, adapting to the developmental needs of diverse populations.
  • Conduct individual, group, and family therapy sessions, focusing on client empowerment and recovery strategies.
  • Collaborate with interdisciplinary teams to ensure a holistic approach to patient care and to uphold the highest ethical standards.
  • Manage a caseload effectively, ensuring that therapy sessions meet the clinical needs and goals outlined by clients.
  • Maintain accurate clinical documentation and timely reporting of client progress and outcomes.
  • Participate in the agency’s quality improvement initiatives and contribute insights for enhancing service delivery.
  • Provide crisis intervention as needed, effectively supporting clients during significant emotional distress.
  • Be available for on-call coverage during program hours to address urgent client needs.
Patient Population / Client Focus

Your primary focus will be seniors and adults on hospice care who often face complex emotional and psychological challenges. Many clients may also struggle with grief, anxiety, and depression, particularly in a hospice setting. Your expertise will help navigate these sensitive dynamics, deliver appropriate support, and facilitate connections to needed resources.

Qualifications
  • Current state licensure as a Licensed Clinical Social Worker (LCSW).
  • Possess a Bachelor’s degree in Social Work; a Master's degree is preferred.
  • Strong verbal and written communication skills to foster productive relationships with clients and colleagues.
  • Demonstrated ability to work collaboratively within a team and engage with clients from various cultural backgrounds.
  • Organizational and planning skills essential for effectively managing multiple clients and therapy sessions.
Preferred Experience
  • A minimum of two years of clinical experience, particularly in a mental health or hospice setting.
  • Familiarity with evidence-based therapeutic practices and crisis intervention techniques.
  • Experience implementing integrated care pathways and working with interdisciplinary teams.
Compensation and Benefits
