    Job description

    Position Purpose:

    This position serves to create a robust referral network through establishing collaborative relationships with agency providers within our region.  This referral network is intended to include agencies that provide services to support a comprehensive wrap around service plan for the client and their family.  This position will work collaboratively with the Rosecrance Pathways to Success program to ensure all identified needs are able to be met through the robust referral network.  This position will maintain the working relationships with the agencies involved in the referral network, and will host quarterly and monthly meetings with those agencies to assess effectiveness of the referral network.  The position will also work with other agencies to market their ability to provide additional Pathways to Success services under HFS guidelines.

    Qualifications/Basic Job Requirements:

    • Bachelor’s Degree in Social Work, Human Services, Public Administration, Sociology, or related field plus experience in mental health services

    • Highly organized with the ability to manage multiple tasks, prioritize responsibilities, and navigate diverse relationships. 

    • Demonstrated ability to work both independently and collaboratively within groups, fostering effective teamwork and initiative. 

    • Proficient in coordinating and leading meetings with both internal and external resources, facilitating communication and collaboration to achieve common goals. 

    • Excellent written and oral communication skills

    • Strong interpersonal skills with the ability to build rapport, establish trust, and form bonds with external agencies. 

    • Competence in using common computer applications such as word processing, spreadsheet, and presentation software

    • Valid driver’s license and ability to meet insurance agency requirements

    • Belief in the mission and vision of Rosecrance 

    Essential Responsibilities:

    1. Work hours prescribed by leadership and any additional hours deemed necessary for the responsibilities assigned.

    2. Develop partnerships with youth-serving organizations.

    3. Actively seek out and participate in existing networking meetings within the community to promote the Pathways to Success mission, demonstrating proactive engagement and commitment to community outreach and partnership-building.

    4. Maintain communications with individuals and agencies involved in the Pathways to Success collaboration.

    5. Develop and host on-going meetings with outside agencies and individuals to present and market the Pathways to Success services available.

    6. Responsible for coordination of the implementation of software systems for managing regional resources.  

    7. Seek out and develop strategies to improve communication about the Pathways to Success resources among area social service agencies and internal Rosecrance departments. 

    8. Attend local area networking meetings.  

    9. Seek out opportunities for further collaboration with the area Community Mental Health Board.

    10. Present workshops, lectures and training to patients, families, and staff and community groups.

    11. Represent the department in PI committees and organizational initiatives as determined appropriate by Director 

    12. Become familiar with all related state and county contracts, and complete monthly reports as required.

    13. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.

    14. Assume other related responsibilities as assigned by management.

    Physical Requirements/Percentage of Work Time

    1. Vision:  Must be able to read clinical documentation, policies and procedures and other written materials  75-100%

    2. Hearing:  Must be able to hear well enough to communicate with patients within an individual, group or family setting  75-100%;  communicate with coworkers 25-49%;  communicate on the telephone with a variety of individuals  25-49%

    3. Standing/Walking/Mobility:  Must have mobility between departments within the facility 25-49%

    4. Climbing/Stooping/Kneeling:  0-24%

    5. Lifting/Pulling/Pushing:  Must exert up to 20 pounds of force occasionally and/or up to 5 pounds of force frequently to lift or move objects  25-49%

    6. Fingering/Grasping/Feeling:  Must be able to finger keyboard for computer work and phone equipment for placing/receiving calls  50-74%

    7. Sitting:  must be able to sit for prolonged periods of time when conducting individual or group counseling, assessments, etc.  50-74%

    Schedule: Monday-Friday 830a-5p

    Starting Pay: Based on education, experience and credentials

    Bachelor’s degree level $46,800
