Physical Therapist
- Job Title: Physical Therapist
- Location: Salida, CO
- Position Type: Contract
- Pay Rate: $55.00-$63.00/hr
- Schedule: Tuesday-Friday, 10-hour day shifts
About the Role
We are seeking a Physical Therapist for a contract opportunity with Heart of the Rockies Medical in Salida, Colorado. This is a strong opportunity for a candidate with outpatient and orthopedic care experience who is looking for a steady rural healthcare clinic assignment in a beautiful Colorado location.
Responsibilities
- Perform comprehensive patient evaluations to assess impairments, functional limitations, and therapy needs
- Develop individualized treatment plans based on diagnosis, prognosis, and patient goals
- Provide outpatient and orthopedic physical therapy services within the PT scope of practice
- Design and modify therapeutic interventions to improve mobility, strength, function, and daily living activities
- Support injury prevention, wellness, fitness, and overall patient health education
- Maintain accurate and timely documentation, including treatment plans, progress notes, physician orders, attendance, and discharge summaries
- Collaborate with healthcare team members to support quality patient outcomes
- Ensure timely completion of therapy documentation and service-related charges
- Requirements
- Minimum 2 years of Physical Therapist experience required
- Outpatient and orthopedic care experience required
- Active Colorado Physical Therapist license required
- Current CPR certification required
- Graduate of an approved physical therapy program
- APTA membership or eligibility preferred
- Ability to work Tuesday-Friday, 10-hour day shifts
- Strong clinical judgment, documentation skills, and patient-centered care approach
