Work Environment

This role is designed to be flexible, providing the opportunity to work remotely while engaging with clients primarily through virtual platforms. As a part of our hospice center, you will be encouraged to participate in regular team meetings, case conferences, and training sessions, all conducted remotely, ensuring seamless collaboration among colleagues.

Key Responsibilities

• Conduct comprehensive assessments to identify the social, emotional, and environmental challenges faced by clients, ensuring tailored support.
• Develop and implement individualized service plans collaboratively with clients, outlining clear goals, interventions, and methods for progress monitoring.
• Provide ongoing counseling and therapeutic support to individuals and families navigating life challenges, utilizing evidence-based practices.
• Engage in crisis intervention, including risk assessments and safety planning to ensure client well-being.
• Coordinate with community organizations and agencies to advocate for clients and connect them with essential resources.
• Participate in community outreach efforts to increase awareness of services available through Unity Support Services, strengthening community ties.
• Actively engage in case conferences, team meetings, and ongoing training to continuously enhance skills and contribute to team goals.

Patient Population / Client Focus

As a Mental Health Therapist at Unity Support Services, you will primarily focus on supporting clients facing complex and diverse challenges arising from various socioeconomic backgrounds. Your role will encompass providing services to individuals experiencing mental health issues, family dynamics, crisis situations, and trauma. The work you do directly correlates to enhancing client outcomes, empowering them to achieve emotional stability, improve relational dynamics, and access necessary community resources.

Qualifications

• Master's degree in Social Work from an accredited university.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of trauma-informed care principles and community resource networks.
• Exceptional communication skills, cultural sensitivity, and the ability to adapt problem-solving approaches to meet diverse client needs.

Preferred Experience

• Prior experience working in a social service agency or an environment focused on mental health care is preferred.

Compensation and Benefits
