Key Responsibilities

As a Licensed Professional Counselor at Trinity Health Behavioral Center, you will be responsible for:

• Assisting primary care providers in identifying and managing mental health and psychosocial issues, ensuring collaborative patient care.
• Conducting patient intakes with a focus on diagnostic and functional evaluations, and providing recommendations for treatment plans to primary care providers.
• Delivering brief, focused therapeutic interventions to patients in need of mental health services.
• Identifying patients who may benefit from specialty mental health referrals, ensuring they receive the appropriate level of care.
• Initiating follow-ups to monitor patient progress and adjusting treatment approaches as necessary.
• Developing relapse prevention plans to help patients maintain stability in their mental health.
• Identifying at-risk patients and devising strategies to prevent further deterioration of their condition.
• Documenting patient progress and therapeutic interventions in clinical records, ensuring compliance with agency and FQHC guidelines.
• Collaborating with the primary care team to ensure cohesive treatment plans and effective communication regarding patient needs.
• Providing education and self-management resources to empower patients in their recovery journey.
• Consulting with clinic management and interdisciplinary team members to enhance program outcomes and service delivery.
• Supporting patients in adhering to prescribed medical treatments and addressing concerns such as medication side effects.
• Upholding the highest ethical standards as guided by the Code of Ethical Conduct for Clinical Professionals.

Qualifications

To be considered for this role, you should possess the following qualifications:

• A Master’s Degree in Clinical Counseling or a related field.
• Current LPC (Licensed Professional Counselor) license in Pennsylvania.

• A strong understanding of psychodiagnostics and the impact of mental health conditions on individuals and families.

Preferred Experience

While not required, candidates who are bilingual in English and Spanish will be given preference, as this will facilitate broader communication with our diverse patient base. Additionally, prior experience working in integrated healthcare settings and familiarity with Federal Qualified Health Center (FQHC) standards would be advantageous.

Trinity Health Behavioral Center offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience.
