Key Responsibilities
  • Assist primary healthcare providers in identifying and managing mental health and psychosocial concerns through collaborative efforts.
  • Conduct comprehensive patient intakes, perform diagnostic evaluations, and recommend personalized treatment plans to primary care providers.
  • Offer brief, focused interventions for patients requiring immediate mental health support, ensuring timely access to care.
  • Monitor patient progress through ongoing follow-ups, adjusting treatment strategies as necessary to achieve optimal outcomes.
  • Develop relapse prevention plans to support patients in maintaining stable mental health.
  • Identify at-risk patients, working proactively to implement interventions that prevent the deterioration of their conditions.
  • Document patient progress meticulously in treatment charts, ensuring compliance with agency and Federally Qualified Health Center (FQHC) standards.
  • Provide education to patients on managing their health conditions and empowering them with self-management skills.
  • Consult with clinic management and team members to enhance behavioral services and contribute to program evaluations and outcomes.
  • Adhere strictly to the Code of Ethical Conduct for Clinical Professionals while performing all duties.

Compensation and Benefits

This full-time position offers a competitive annual salary ranging from USD 87,000 to 105,000, based on qualifications and experience.
