Work Environment

Key Responsibilities
  • Conduct comprehensive assessments to accurately identify the social, emotional, and environmental challenges faced by clients, utilizing standardized assessment tools.
  • Develop and implement personalized service plans in collaboration with clients, including specific goals, actionable interventions, and consistent progress monitoring.
  • Provide evidence-based counseling and therapeutic support to individuals and families navigating significant life challenges, including crisis situations.
  • Engage in active client advocacy, facilitating connections to community resources and services tailored to their unique circumstances.
  • Deliver timely crisis intervention and conduct risk assessments, creating safety plans that prioritize client well-being.
  • Coordinate with community organizations and agencies to streamline access to services and resources for clients.
  • Participate in community outreach efforts to raise awareness of services provided and foster partnerships that enhance community support.
  • Contribute to team collaboration through participation in case conferences, staff meetings, and ongoing training sessions geared towards professional development.
Patient Population / Client Focus

The ideal candidate will focus on diverse populations, including families and individuals experiencing homelessness, mental health issues, domestic violence, and substance use disorders. You will play a pivotal role in supporting these clients through personalized interventions aimed at empowering them to overcome barriers and achieve long-term stability.

Qualifications
  • A Master’s Degree in Social Work from an accredited institution is required.
  • Current active state licensure as a Licensed Clinical Social Worker (LCSW) is necessary for this role.
  • Strong understanding of social and economic justice issues, with the ability to apply this knowledge within client interactions and case management.
  • Excellent communication and interpersonal skills, with a demonstrated ability to connect with clients from diverse backgrounds.
  • Competency in problem-solving and critical thinking to effectively navigate client needs and service delivery.
Preferred Experience
  • A minimum of two years of experience working in a social service agency or comparable environment.
  • Familiarity with trauma-informed care practices and techniques, including skills in crisis intervention.
  • Proficient use of case management software and remote communication tools to maintain effective client interactions.
Compensation and Benefits
