Position Overview

At Liberty Healthcare and Rehabilitation Services, we promote a challenging yet rewarding opportunity in a caring environment. We are currently seeking a PHYSICAL THERAPIST to work with a multidisciplinary team to ensure a wonderful rehab experience for patients and their families by being responsive and individualizing treatment plans. Be able to work in a supportive rehab environment with progressive treatment modalities to address a variety of diseases and impairments. In-house therapy with local management support.

Responsibilities

• Work with a multidisciplinary team to provide individualized treatment plans for patients.
• Respond to patients and families with individualized care and support.
• Implement progressive treatment modalities for a variety of diseases and impairments.
• Collaborate with in-house therapy and local management for effective patient care.

Qualifications

• Graduate of an accredited therapy program.
• Licensed in the state of the SNF where the position is located.
• Preferable prior experience in a skilled nursing facility rehabilitation setting.
