Key Responsibilities
  • Conduct thorough clinical assessments of patients to determine their psychosocial needs and develop customized treatment plans that address their emotional and mental health challenges.
  • Administer evidence-based therapeutic interventions consistent with each patient’s treatment goals, utilizing methodologies such as cognitive-behavioral therapy (CBT) or mindfulness techniques.
  • Maintain accurate, detailed clinical records and documentation in compliance with state regulations and organizational policies, ensuring that patient confidentiality is upheld.
  • Collaborate closely with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to devise integrated care strategies tailored to individual patient needs.
  • Provide education and ongoing support to patients and their families, enhancing their understanding of mental health issues and the resources available to them.
  • Stay informed about current trends, research developments, and best practices in mental health care to ensure the highest standard of treatment is provided.
Patient Population / Client Focus

You will work primarily with patients facing terminal illnesses and their families, a population that requires sensitivity, empathy, and skilled therapeutic interventions. Qualifications
  • A minimum of a Master’s degree in Social Work from an accredited institution, ensuring a strong theoretical foundation in social work practices.
  • A valid LCSW license in the state of New York, allowing for the legal practice of clinical social work.
  • Proven knowledge of solution-focused psychotherapy interventions that can be effectively applied to a diverse array of patient scenarios.
  • Exceptional communication, problem-solving, and organizational skills, emphasizing the ability to build rapport with patients and collaborate with colleagues.
Preferred Experience
  • Experience working within a hospice or palliative care setting, particularly in delivering mental health services to patients and families facing bereavement.
  • Familiarity with electronic health record (EHR) systems to streamline clinical documentation and coordinate care effectively.
  • Enhanced skills in trauma-informed care, addressing the complex needs of clients with diverse backgrounds and experiences.
