Key Responsibilities
  • Conduct initial psychosocial assessments to evaluate clients' needs and establish appropriate intervention plans that align with hospice care goals.
  • Provide ongoing clinical supervision and support to team members and practitioners delivering behavioral health interventions, ensuring adherence to clinical standards and policies.
  • Facilitate crisis intervention for clients facing critical issues such as mental health emergencies, familial disruptions, or other traumatic experiences.
  • Advocate for community resources that support patient well-being, such as healthcare services, financial assistance, and social services.
  • Follow up with clients to monitor progress and adjust care plans as necessary, ensuring a responsive approach to changing needs.
  • Implement therapeutic interventions, including individual, family, and group therapy sessions via telehealth platforms.
  • Collaborate with interdisciplinary teams to provide comprehensive support in hospice care, enhancing overall patient outcomes.
Patient Population / Client Focus

Your primary focus will be on patients receiving hospice care, including high-risk populations and individuals coping with terminal illnesses. Additionally, you will support their families through the grief process and help them navigate the many complexities associated with end-of-life care. Your expertise in working with veterans and providing specialized support will be essential in addressing their unique needs.

Qualifications
  • Possession of a Master's Degree in Social Work from an accredited university.
  • A valid California Licensed Clinical Social Worker (LCSW) certification is mandatory.
  • A minimum of 2 years of experience in a behavioral health setting, preferably in hospice or palliative care.
  • Demonstrated ability to perform detailed psychosocial assessments, including interview techniques and risk evaluations.
  • Strong knowledge of community resources, particularly those relevant to hospice care and patient support.
  • Exceptional communication and interpersonal skills, enabling effective collaboration with patients, families, and healthcare teams.
Preferred Experience
  • Experience working with veteran populations or within military health systems.
  • Familiarity with electronic health record (EHR) systems to manage patient documentation efficiently.
  • Proficiency in crisis intervention techniques and therapeutic modalities.

The position offers a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and education.
