Rooted Life LLC is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team in a full-time remote capacity. Work Environment

This position is primarily remote, allowing you to conduct assessments and provide therapy from the comfort of your own home. However, you will remain closely connected to our hospice team to ensure collaborative patient care. Key Responsibilities

• Conduct comprehensive assessments for newly admitted patients, accurately identifying strengths, weaknesses, psychosocial needs, and goals during intake.
• Utilize evidence-based therapeutic modalities tailored to individual patient needs, fostering a supportive therapeutic relationship.
• Design specific psychological interventions that empower clients to achieve their personal and treatment goals, adapting strategies as necessary.
• Provide individual and family counseling services, focusing on grief, loss, and adjustment to terminal conditions.
• Engage in crisis intervention when necessary, ensuring that appropriate documentation and follow-up actions are taken to safeguard patient welfare.
• Collaborate with community resources and agency partners to coordinate comprehensive care, enhancing service access for clients and their families.
• Offer telephonic support and guidance to families and patients as needed, demonstrating responsiveness to dynamic emotional needs.
• Maintain active licensure in accordance with state requirements and stay updated on best practices in social work.
• Provide feedback and insights to the treatment team regarding patient progress, ensuring continuity and quality in care delivery.

Patient Population / Client Focus

In this role, you will primarily work with patients facing end-of-life challenges and their families, addressing complex emotional and social issues that arise during this sensitive time. Your expertise will guide families through the grieving process and provide necessary resources to ensure they feel supported and equipped to handle their circumstances. Understanding the unique needs of diverse populations will be critical in personalizing care for each individual.

Qualifications

• A Master's Degree in Social Work from an accredited institution.
• Current Florida Behavioral Sciences Licensure as a Licensed Clinical Social Worker (LCSW).
• Certification in Basic Life Support for Healthcare Professionals, issued by the American Heart Association (AHA).
• Strong organizational skills and critical thinking capabilities.
• Proficiency in counseling techniques, assessment methodologies, and effective interviewing skills.

Preferred Experience

• A minimum of two years of experience providing social work services in a behavioral health or hospice setting, ideally with direct patient and family interaction.
• Experience in crisis intervention, particularly in the context of end-of-life care or complex emotional situations.
