Position Overview

Riverplace Health System is seeking a dedicated and dynamic Licensed Clinical Social Worker (LCSW) to join our compassionate hospice team. Work Environment

This position is fully remote, allowing our LCSWs to manage their schedules while working from the comfort of their homes. Our state-of-the-art virtual care platform facilitates telehealth appointments, enabling seamless interaction with patients. The role fosters a collaborative atmosphere where LCSWs connect with healthcare professionals, including physicians and nursing staff, to deliver coordinated care for our patients. Team meetings and supervision sessions are held weekly via video conferencing to maintain strong communication and support.

Key Responsibilities

• Conduct comprehensive assessments to evaluate patients' mental health status, emotional well-being, and social functioning, utilizing evidence-based frameworks.
• Develop individualized treatment plans that are culturally competent and responsive to the unique challenges faced by hospice patients, including grief, anxiety, and end-of-life concerns.
• Provide psychotherapy, utilizing therapeutic modalities tailored to each patient's needs, such as cognitive-behavioral therapy or supportive counseling.
• Facilitate support groups for patients and their families, providing a safe space to share their experiences and feelings.
• Collaborate with a multidisciplinary team, including medical professionals, to ensure a holistic and coordinated approach to patient care.
• Regularly monitor patient progress, making timely adjustments to treatment plans based on feedback and clinical developments.
• Deliver educational programs for patients and families that cover mental health conditions, available resources, and coping strategies during the hospice process.
• Adhere strictly to ethical practices, maintaining patient confidentiality and informed consent protocols.

Patient Population / Client Focus

This role focuses on providing support to patients experiencing terminal illnesses and their families. You will work with individuals facing complex psychological challenges related to grief, fear, and depression, ensuring that emotional support is integral to their care. You will also play a crucial role in empowering families during the transition process, helping them access resources and provide necessary support to their loved ones.

Qualifications

• Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of 3 years of experience in providing psychotherapy and mental health services, particularly in hospice, palliative care, or chronic illness settings.
• Exceptional communication and interpersonal skills to foster strong relationships with patients, families, and interdisciplinary team members.
• Proficient clinical skills, including the ability to conduct thorough assessments and craft personalized treatment plans based on each patient's unique situation.
• Experience with electronic health records (EHR) and telehealth technology to manage patient documentation and facilitate remote care.
• In-depth knowledge of mental health conditions, treatments, and a commitment to providing culturally competent care.

Preferred Experience

• Experience in hospice or palliative care settings.
• Familiarity with community resources for mental health and end-of-life care support.
• Background in grief counseling or support services.

Compensation and Benefits
