LifeScape is seeking an interested candidate to join our dynamic and collaborative team as an external Case Manager supporting adults contracted externally from various Community Support Providers.

As a valued member of LifeScape's disciplinary team, you will be responsible to assess and advocate with and on behavior of the individuals supported in all respects. In addition to advocacy, working hand in hand with families and team members you will maintain regular contact to promote growth and positive well-being for individuals on your caseload. You will have the ability to advance current programs, as well as collaborate to develop new programs.
On an ongoing basis, this position will support LifeScape's vision to be an innovative organization, providing exceptional services and creative solutions for people with varied needs and requiring complex care across their lifespan by demonstrating our values of integrity, compassion, respect, safety and well-being, fiscal responsibility, excellence, and accountability.
KEY RESPONSIBILITY AREASFacilitate, develop, and monitor participants' provision of services, health, and welfare
  • Assists participant in finding and selecting services and supports, both paid and unpaid natural supports
  • Assists in accessing supports to transition from an institutional setting, the family home or from one provider to another
  • Assists individuals to access integrated community employment and ensure person-centered employment planning
  • Initiates a comprehensive assessment and periodic re-assessment of the individual needs to develop, revise and update the participant's ISP
  • Observes, monitors, and documents the implementation of the ISP at least quarterly
    • Intervenes as necessary to ensure implementation and revisions of the ISP
    • Provides quarterly report to all team members, including guardians
  • Reviews Human Rights Committee (HRC)/ Supportive Approaches Committee (SAC) findings to ensure due process has been afforded
  • Identify and report emergency rights restrictions to HRC/SAC and the ISP team within 24 hours/or the next business day, per Administrative Rules of South Dakota (ARSD)
  • Assesses individual eligibility status and refers individual to necessary eligibility resources, ensures the completion of all necessary paperwork to obtain, and maintains eligibility and funding
Requirements
Education and Experience
Bachelor's degree in a behavioral science, social science, education, or related field is required. Two years of experience working with people with intellectual or physical disabilities required; more than four years preferred. Valid driver's license; clean driving record. Successful completion and certification in training provided by agency within specific time allotment required.
All positions require a clear background check and drug screen upon conditional offer.
