Overview

The Program for All-Inclusive Care for the Elderly (PACE) is a Medicare and/or Medicaid/Medi-Cal-funded program that helps eligible older adults meet their health needs in the community rather than going to a nursing home. With roots in California since the 1970s, PACE has delivered superior outcomes in quality, longevity, and care experience.

Habitat Health is an innovative collaboration with Kaiser Permanente with the goal of empowering older adults to remain independent by providing the care and support they need to live safely in their own homes and communities as they age. The program provides comprehensive insurance, medical, and social support services in centers in the community and extending into participants- homes. Kaiser Permanente serves as the main specialist and hospital network for Habitat Health-s current programs, giving all Habitat Health participants access to Kaiser Permanente-s diagnostic and laboratory services, hospitals and acute care facilities, and outpatient specialty services. Habitat Health redefines aging in place to bring more good days and a lasting sense of belonging to participants and their caregivers.

The Provider Network Managerial Consultant V provides operational leadership for Kaiser Permanente in support of the provider network for Habitat Health, serving as the network liaison between Kaiser Permanente operations and Habitat Health. This role advances Kaiser Permanente's provider network from manual processes to sustainable, standardized operations that ensure a highly reliable model of care.

The role manages day-to-day operations and develops operational systems, dashboards, and processes that deliver safe, timely, and coordinated care for Habitat Health participants - including specialized workflows to optimize KPHC chart accuracy, revenue cycle management, issue resolution, and provider network expansion.

The role also drives structured expansion of the Habitat Health-Kaiser Permanente collaboration through coordinated workflow and process development, with a focus on highly reliable systems and standard operations. Responsibilities include leading team and systems readiness for future site launches and contributing to a standardized operations plan for scalable, replicable growth.

Job Summary:

Serves as a lead consultant to ensure the alignment, buy-in, and coordination of diverse stakeholders to drive the implementation of strategic business initiatives and projects/programs across multiple functional tracks or workstreams. Drives business strategy, organizational alignment, and prioritization of business initiatives. Leads the work of project/program team members. Provides expertise and drives the development and implementation of business initiatives, systems, and/or processes to a desired future state. Manages complex, cross-functional projects/programs, and serves as an expert in the proactive monitoring and identification of client, project, program, and/or business risks. Designs, leads, and serves as an expert for change management plans associated with business initiatives. Leads assessment of strategic performance metrics to support business initiatives. Participates in vendor management as required. Monitors compliance of work activities by ensuring business plans and team members adhere to relevant policies and procedures.

Minimum Qualifications:

  • Minimum two (2) years experience in a leadership role with or without direct reports.
  • Bachelors degree from an accredited college or university and Minimum eight (8) years experience in consulting, project management, data analytics, operations or a directly related field OR Masters degree in Business Administration, Public Health, Health Services Administration, or related field and Minimum six (6) years experience in consulting, project management, data analytics, operations or a directly related field OR Minimum eleven (11) years experience in consulting, project management, data analytics, operations or a directly related field.

Preferred Qualifications:
