Job description

Compensation Range: $17/hr - $20/hr commensurate with experience
Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.
Overview:
Provide vocational rehabilitation counseling services to individuals (and families) underserved who are experiencing homelessness, substance use, and/or co-occurring mental health disorders directly in the community. Grant funds by SAMHSA via Intercambios Puerto Rico (Fajardo, Puerto Rico).
Responsibilities:
  1. Screen individuals (mostly in the community) for eligibility of services.
  2. Use the supported employment model as an integrated part of mental health treatment.
  3. Advise participants on personal benefits related to employment status.
  4. Link program participants to support services via referrals.
  5. Know and promote evidence-based practices (mostly Motivational Interview, Cognitive Behavioral Therapy and Harm Reduction)
  6. Prepare an individualized employment support services plan for eligible participants.
  7. Prepare participants for job interviews.
  8. Complete SUPRT tool and vocational interest assessment.
  9. Design and implement a job marketing plan for eligible participants.
  10. Document and update the participant record and report data on required systems on establish timeframe.
  11. Conduct group and individual educational activities on the topics covered by the project.
  12. Provide continuous support and follow up services to program participants according to the timeframe. Carry out the processes in compliance with the rules, procedures, and protocols of the organization.
  13. Visit organizations and services providers to create collaboration relations for the benefit of the GBHI Project
  14. Prepare monthly progress report.
  15. Maintain an updated list of participants and identify active and inactive participants.
  16. Accompany and assist the participant in any process if necessary.
  17. Participate in staff meetings and case discussions, as required.
  18. Driving the official or rented vehicle when necessary to complete project tasks.

Requirements:
  • Experience with providing rehabilitation counseling services to people that have experienced homelessness, substance use, and/or co-occurring disorders.
  • Knowledge and/or experience with Evidence Based Practices / Interventions
  • Proficient and efficient with multiple computer software programs
  • Meticulous at maintaining records and confidentiality protocols.
  • Good written and verbal communications skills (in both Spanish and English a plus)
  • Able to work closely with others in outreach team and office setting (team player)
  • Up to date driver's license
  • Master in Rehabilitation Counseling
