Our team values autonomy and collaboration, fostering an environment where clinicians can thrive professionally while delivering high-quality care.

Key Responsibilities
  • Conduct thorough assessments to evaluate clients’ psychological and emotional states, ensuring a comprehensive understanding of each individual’s needs and treatment objectives.
  • Facilitate diverse therapeutic modalities, including individual, couples, and family therapy sessions, utilizing evidence-based approaches such as cognitive-behavioral therapy, mindfulness practices, or narrative therapy depending on the client’s needs.
  • Develop, implement, and monitor personalized treatment plans, ensuring regular updates based on clients’ progress and feedback.
  • Engage clients in collaborative goal-setting processes, reinforcing their agency and involvement in their therapeutic journey.
  • Maintain rigorous standards of confidentiality and adhere to ethical practices and legal regulations in the mental health field.

  • Provide emotional support and practical coping strategies to clients as they navigate life transitions and challenges associated with the hospice care process.
Patient Population / Client Focus

In this role, you will work primarily with hospice patients and their families, providing support during a critical and often emotionally charged period of life. You will help families navigate the intricacies of palliative care, ensuring they feel understood and supported throughout their journey.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Demonstrated clinical skills in assessment, intervention, and treatment planning relevant to hospice care.
Preferred Experience
  • A minimum of two years of clinical experience in a mental health setting, particularly with hospice patients or similar vulnerable populations.
  • Experience in private practice or a related mental health environment, where personalized care and attention have been key focuses.
  • Outstanding communication and interpersonal skills, with a strong ability to engage clients empathetically.
