Job description

Nurse Practitioner

Position Summary: Under collaboration of the Consultant Family Physician, or his/her designee, provide assigned nursing and medical services within the limits and scope of training and license.

Essential Duties/Responsibilities:

  • Obtain relevant physical health history for assigned patients
  • Evaluation and treatment of general/medical illnesses under the collaboration of the physician
  • Performs other duties as assigned by the Chief Medical Officer
  • Assume primary on-call responsibilities with back-up provided by the collaborating physician. This duty includes the provision of outpatient primary care services
  • Will take care of walk-in cases and tele-med care as needed
  • Flexibility in work schedule as determined by organizational needs
  • Provision of data and quality metrics as required by FQHC Program of HCI

Minimum Qualifications/Requirements:

  • R.N. with a M.S. Degree as a Nurse Practitioner or Clinical Nurse Specialist
  • Board certification as a Family Nurse Practitioner or Clinical Nurse Specialist
  • Competency in diagnoses and treatment of the general medical guidelines
  • Willingness to use personal transportation in work capacity
  • Active collaboration with referrals to community physicians for quality care for HCI consumers

Certificates, Licenses, Registrations:

  • Current and active Indiana License as Registered Nurse
  • Current and active Indiana License as a Nurse Practitioner or Clinical Nurse Specialist
  • Current and active Indiana Controlled Substances Registration Certificate
  • Board certification as a Family Nurse Practitioner or Clinical Nurse Specialist
  • Current and active DEA certificate
  • Maintain current American Red Cross CPR/AED certifications.
  • Maintain current Crisis Prevention Intervention (CPI) certification.
  • Valid Driver's License in accordance with motor vehicle policy
  • Completes all required continuing education requirements in a timely manner.

Physical Demands:

  • Standing, walking, bending, stooping as required to provide individualized patient care.
  • Possesses strength necessary to direct and assist in implementation of CPI interventions.

Work Environment:

  • Most of the work of the nurse practitioner is done directly with consumers in Center facilities, which provide a comfortable physical environment with consumers. On occasion, the nurse practitioner may see clients either in their homes or in community settings. Office and program areas may not be air-conditioned and could become overly warm in summer months. Work conditions, such as cleanliness, comfort and safety, may vary widely. Evening and/or weekend work may be required. Smoking is permitted only outside of program and office sites. Safety management and crisis intervention decisions and actions are dependent on staff present. Crises may arise which may be stress producing and/or be exhausting.

Conditions of Employment:

  • Completion of tuberculin screening no later than three days prior to first day of employment and annually thereafter
  • Completion of MMR, Varicella, influenza and coronavirus vaccine
  • Completion of a pre-employment drug screening and completion of post-employment drug or alcohol tests upon reasonable suspicion of use.
  • Satisfactory completion of an Indiana Department of Child Services criminal fingerprinting background, Indiana State Criminal History, Sex and Violent Offender Registry, Child Protection Services History, and local law enforcement agency checks at the time of hire and every four years (or at contract renewal when applicable) for any employee that has direct contact on a regular and continuing basis with DCS consumers.
  • Completion of Center-wide orientation and ALL required paperwork prior to reporting for work
  • Demonstrated computer literacy through successful completion of pre-employment testing may be required.
  • Completion of HCI Commitment to Quality training, Trauma Informed Care, and E-Learning
  • Attendance at all mandatory staff development and training
  • Successful completion of a six month on the job orientation period
  • Participation in payroll electronic deposit
  • Adherence to Compliance Program Plan
  • Satisfactory reference and background investigation checks.
