Work Environment

This full-time position offers the flexibility of a remote work setting, allowing you to connect with clients through digital platforms while maintaining the personal touch necessary for effective social work. Typical work hours are Monday to Friday from 8:00 AM to 4:30 PM, although occasional evening or weekend hours may be required to meet client needs. As part of this role, you will also need to demonstrate flexibility in scheduling to ensure continuous care and support. In some cases, you may be asked to be available on-call to address urgent situations or provide necessary assistance during off-hours.

Key Responsibilities
  • Conduct comprehensive assessments of each client's psychosocial needs by analyzing emotional, mental, and social factors that affect their well-being.
  • Develop collaborative care plans in partnership with clients and their families, tailoring interventions to their specific goals and preferences.
  • Provide ongoing counseling and emotional support, guiding seniors and their families through life transitions and prevailing challenges.
  • Advocate effectively for client rights and coordinate access to essential community resources, ensuring they receive the support necessary for their circumstances.
  • Work collaboratively with interdisciplinary teams, including nurses and healthcare professionals, to enhance delivery and quality of care.
  • Perform regular follow-up visits to monitor client progress, evaluate living conditions, and address any emerging concerns or safety issues.
  • Facilitate communication between family members involved in the senior client's care, helping to resolve conflicts and improve collaborative care efforts.
  • Educate clients and their families about available community resources, support groups, and programs aimed at increasing social engagement and overall life satisfaction.
  • Maintain thorough documentation of client evaluations, care plans, progress reports, and significant interactions, adhering to regulatory and ethical standards.
  • Stay abreast of the latest developments in social work practices, advancing your knowledge and ensuring compliance with established policies and standards.

Qualifications

Compensation and Benefits

This full-time Licensed Clinical Social Worker position offers a competitive salary range of USD 70,000 to 100,000 annually, commensurate with experience and expertise.
