Work Environment

This role is designed as a remote position, allowing for flexibility while operating in a hospice care framework. Key Responsibilities

• Conduct individual and group therapy sessions tailored to the specific emotional and psychological needs of hospice patients and their families.
• Utilize evidence-based counseling techniques to effectively address a range of mental health concerns, including anxiety, depression, grief, and loss.
• Maintain comprehensive and accurate clinical documentation, ensuring compliance with legal and ethical standards in social work practice.
• Collaborate with interdisciplinary teams to facilitate case discussions, patient care planning, and referrals to additional services as needed.
• Engage in ongoing assessment of patients’ progress and adapt treatment plans accordingly to meet their evolving needs.
• Facilitate family meetings to promote open communication and support for family members dealing with complex emotional challenges.

Patient Population / Client Focus

The Licensed Clinical Social Worker in this role will primarily work with individuals facing terminal illnesses, along with their families. Qualifications

• A Master’s degree in Social Work (MSW) from an accredited institution is required.
• Current and active LCSW licensure in the state of Pennsylvania.

• Proficiency in medical documentation processes and electronic health record (EHR) systems.

Preferred Experience

• Training in motivational interviewing techniques to better engage and support clients in therapy.
• Specialization in pediatric social work or experience with teens in mental health contexts.
• Familiarity with crisis intervention practices and conflict resolution strategies.

Compensation and Benefits
