If you thrive in a collaborative environment and are eager to make a significant impact on patient recovery and independence, we encourage you to apply.

About the Organization

Forward Function Healthcare Center is a reputable provider of rehabilitation services in Corvallis, known for our commitment to patient-centered care. Our facility specializes in helping individuals overcome challenges posed by physical and cognitive impairments, allowing them to reclaim their independence. Our team of skilled professionals works closely with patients, families, and multidisciplinary teams to create a supportive and effective rehabilitation journey.

Work Environment

Located in a vibrant community, our rehabilitation facility combines modern equipment with a welcoming atmosphere. As an onsite Occupational Therapist, you will work directly with residents in a clinical setting designed to facilitate recovery and provide personalized care. Our approach emphasizes teamwork, where every member plays a crucial role in delivering exceptional care and fostering patient motivation to achieve their therapeutic goals.

Key Responsibilities
  • Conduct comprehensive evaluations to assess residents’ functional capacities and develop tailored occupational therapy plans that meet their specific needs.
  • Implement and lead therapy sessions, guiding patients through exercises and activities that improve their mobility, self-care, and overall daily living skills.
  • Regularly monitor and document patient progress, making necessary adjustments to therapy plans to adapt to evolving challenges or improvements.
  • Collaborate with other healthcare professionals, including physical therapists and physicians, to ensure a holistic approach to patient care.
  • Maintain compliance with all relevant industry guidelines and regulations while upholding best practices in therapy delivery.
Patient Population / Client Focus

In this role, you will primarily work with adults in a rehabilitation setting, many of whom are recovering from surgeries, injuries, or age-related conditions that affect their ability to perform daily living activities independently. Your work will focus on enhancing their physical and cognitive abilities, ultimately leading to improved quality of life and functional independence.

Qualifications
  • A Bachelor's Degree in Occupational Therapy from an accredited program.
  • Current and active Occupational Therapist license in the state of Oregon.
  • Minimum of two years of experience working in a healthcare setting, preferably in rehabilitation or assisted living.
  • Strong familiarity with therapeutic techniques and modalities, as well as electronic medical record (EMR) systems to ensure accurate patient documentation.
Preferred Experience
  • Experience working within a multidisciplinary team to design and implement care plans.
  • Knowledge of specific rehabilitation tools and assistive technologies.
  • Demonstrated ability to engage and motivate clients through positive reinforcement and collaborative goal setting.
Compensation and Benefits

The salary range for this position is $90,000 to $100,000 annually, commensurate with experience and qualifications. In addition, we offer a comprehensive benefits package, including health, dental, and vision insurance, as well as disability insurance. Our employees also enjoy paid time off, a retirement savings plan, and opportunities for ongoing education and professional development.
