Key Responsibilities

• Engage in regular communication with physicians and healthcare professionals to provide updates on patient progress, identify needs, and address concerns effectively.
• Maintain meticulous records of clinical assessments and progress notes, adhering to departmental policies and ensuring confidentiality.
• Participate actively in forming treatment plans, including discharge strategies, rooted in comprehensive psychosocial assessments and best-practice social work techniques.
• Provide counseling and crisis intervention to patients, utilizing effective strategies to assess and address emotional and behavioral problems.
• Collaborate with families and support systems to formulate treatment directions and discharge plans, ensuring a holistic approach to care.

Patient Population / Client Focus

The LCSW will work with diverse populations within the hospice setting, focusing on helping clients navigate grief, emotional distress, and transitions associated with terminal illness. You will play a vital role in providing comfort and counseling amid life-altering circumstances, ensuring that patients and their families receive compassionate and comprehensive support tailored to their individual journeys.

Qualifications

• Possession of a Master’s degree in Social Work, Counseling, or a related field, guaranteeing a strong educational foundation.
• Current and active license as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania.
• A minimum of 3 years of demonstrated experience in an outpatient setting, particularly within a mental health or hospice care environment.
• Proficient in the use of technology, including electronic medical records, Microsoft Office, and other relevant software necessary for documentation and communication.
• Ability to work independently with high levels of organization, while also thriving in a team-based environment.

Preferred Experience

While the essential qualifications outline the mandatory attributes, additional experience in specialized care settings, knowledge of bereavement processes, and expertise in group therapy modalities will be viewed favorably. Compensation and Benefits
