Work Environment

This position is fully remote, allowing you to work from the comfort of your own home while maintaining your professional work-life balance. You will access innovative tools and software designed to assist social workers in tracking and reporting client care. Your flexible schedule will involve standard work hours from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evenings or weekends based on client needs.

Key Responsibilities
  • Conduct comprehensive assessments to evaluate the psychosocial needs of seniors, recognizing emotional, mental, and social factors affecting their well-being.
  • Develop individualized care plans in collaboration with clients and their families, ensuring that each plan aligns with their specific goals, preferences, and needs.
  • Provide direct counseling services, offering emotional and psychological support to seniors as they navigate life transitions such as end-of-life care, loss, or major changes in their living situations.
  • Advocate for seniors' rights and facilitate access to community resources, ensuring clients and their families receive the necessary support and services to promote their quality of life.
  • Maintain detailed records and documentation in compliance with organizational policies and regulatory requirements, utilizing electronic health record systems to track client progress.
  • Engage in proactive communication with medical professionals, family members, and additional service providers to coordinate comprehensive care.
Patient Population / Client Focus

As a Licensed Clinical Social Worker, you will primarily serve a diverse group of seniors and their families within a hospice care setting. Your focus will be on helping clients experiencing various challenges related to aging, such as chronic illness management, end-of-life planning, and emotional distress. This role requires sensitivity to the unique needs of individuals in their later stages of life and the ability to provide support in a respectful and dignified manner.

Qualifications
  • A master's degree in Social Work from an accredited institution is essential.
  • A current and valid state licensure as a Licensed Clinical Social Worker (LCSW) is required.
  • A minimum of two years of related experience, preferably in a community center, hospice, or geriatric setting.
  • Deep knowledge of social work principles, interventions, and community resources relevant to aging populations.
  • Exceptional assessment, counseling, and problem-solving skills, with a proven track record of developing effective care plans.
Preferred Experience
  • Experience working within a hospice care environment.

Compensation and Benefits
