Work Environment

This position primarily operates within a remote setting, combining the flexibility of telehealth with the personal touch of in-person interactions at our partnership hospice center. Key Responsibilities

As a Licensed Clinical Social Worker at Crystal Psychotherapy, you will have the opportunity to engage with patients and their families through the following responsibilities:

  • Conduct comprehensive assessments for individuals and families to identify psychosocial factors affecting their health and well-being.
  • Provide individual and group therapy sessions, addressing various issues such as alcohol and substance abuse, grief and loss, medical challenges, and family dynamics.
  • Collaborate closely with the clinical team to develop and implement personalized treatment plans that cater to the unique needs of each patient.
  • Facilitate family therapy sessions to improve communication and resolve conflicts within families, addressing issues such as sibling rivalry, blended family dynamics, and adjustment to separation or divorce.
  • Maintain accurate and timely documentation of patient interactions, treatment plans, and progress notes in compliance with regulatory standards.
  • Support referrals to other health services or resources as needed, ensuring holistic patient care.
Patient Population / Client Focus

Your role will encompass a diverse and multifaceted patient population within the hospice center and community settings. You will work with adults and their families facing end-of-life concerns, grief, and trauma associated with serious illnesses. Additionally, you'll provide asset-based solutions tailored to children and adolescents dealing with family disruptions, emotional challenges, and behavioral issues.

Qualifications

To qualify for this position, candidates must possess the following:

  • A valid and active state license as a Licensed Clinical Social Worker (LCSW).
  • A Bachelor's degree in Social Work, preferably with additional qualifications or experience relevant to mental health care.
  • Strong commitment to ethical practices and a genuine passion for improving patient outcomes.
Preferred Experience
