    Job Summary :

    The primary responsibilities of the Home Health Director of Clinical Services include managing and supervising interdisciplinary teams to ensure high-quality patient and family care, overseeing care plans in alignment with organizational philosophy and regulatory standards, auditing documentation for accuracy, and ensuring efficient communication with physicians and external stakeholders. The role also involves staff management, administrative oversight, and occasionally serving as an RN Case Manager or acting Executive Director to support census fluctuations or cover absences.

    How you'll make an impact as a Home Health Director Clinical Services (Registered Nurse/RN)

    • Supervise patient and family care as specified by the plan of care, assessing appropriateness, continuity, service, and quality of care
    • Direct and manage Interdisciplinary Team (IDT) including scheduling, productivity, mentoring and monitoring, 1 on 1's, pay practices and timekeeping.
    • Oversee the consultative process between the Primary Care Physician and the members of the Interdisciplinary Team (IDT).
    • Ensure adherence to the rules and regulations of state and federal regulatory agencies
    • Attend/Lead Interdisciplinary daily team huddle calls.
    • Process EMR documentation workflow.
    • Reviews IDT documentation for accuracy and completeness.
    • Directly supervises up to 45 team members. Carries out supervisory responsibilities including interviewing, hiring, training and terminating (with administrative approval) all interdisciplinary staff; planning, assigned, and directing work; appraising performance; rewarding and disciplining team members; addressing complaints and resolving problems.
    • Promote excellent patient care to support 5 STAR ratings.

    Home Health Director Clinical Services (Registered Nurse/RN) Requirements

    • Must be a Registered Nurse licensed in the state of employment.
    • One (1) year of home health care experience required.
    • One (1) year of supervisory experience preferred.
    • A valid driver's license and auto liability insurance.
    • Current CPR certification.
