Position Overview

Heal California Wellness Services is seeking a dedicated and experienced Senior Licensed Clinical Social Worker (LCSW) to join our compassionate team. This full-time, remote position allows you to deliver essential social work services to our diverse community in Fontana, California. Work Environment

This role is predominantly remote, providing flexibility while remaining engaged with both our team and patients through virtual platforms. You will collaborate with a multidisciplinary healthcare team and utilize advanced electronic health record (EHR) systems to manage patient information. Key Responsibilities

• Document and maintain accurate and comprehensive patient records, including assessments, treatment plans, and case notes, in accordance with ethical guidelines and practice standards.
• Conduct thorough psychosocial assessments to identify the medical, social, and emotional needs of patients and their families, using effective interviewing techniques and collaboration with healthcare team members.
• Develop and periodically refine individualized treatment plans based on the ongoing assessments of patient concerns, strengths, and needs, addressing their unique spiritual and cultural values.
• Utilize evidence-based practices, including behavior modification and supportive therapies, to support patients and families in navigating their challenges and achieving their goals.
• Serve as a liaison with community resources, coordinating services that facilitate access to support systems and enhance patient care continuity.
• Maintain proficiency in the current DSM diagnostic criteria to accurately identify and diagnose mental health disorders in children and adolescents, fostering resilience and growth.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, focusing on individuals and families facing challenges related to mental health, trauma, and social injustice. Your expertise will guide interventions that prioritize both immediate concerns and long-term wellness, developing strong rapport and trust as you help patients navigate their personal journeys.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current, valid license as a Clinical Social Worker (LCSW) in California.
• Exceptional written and verbal communication skills to effectively convey complex information to patients, families, and team members.
• Demonstrated ability to establish and maintain appropriate boundaries within therapeutic relationships.
• Skilled in timely completion and accurate submission of documentation pertaining to patient services.

Preferred Experience

• A minimum of three years of experience as an LCSW in clinical settings, particularly in community health or outpatient environments.
• Experience working with underrepresented populations, including an understanding of cultural competence in treatment.
• Familiarity with various therapeutic modalities and evidence-based practices applicable to diverse patient needs.

Compensation and Benefits

We offer a competitive salary in the range of USD 120,000 to 130,000 annually, commensurate with your experience and caseload management.
